Request Forms

Build the form ministries use when they need something promoted

Request forms are how youth, kids, worship, and other ministries request that comms promote something in Requests. You choose a starting template, add fields, optionally Link answers to campaign fields, then Publish. Members fill the live form; results land in Inbox.

Plan: Forms are part of Requests on Plus. Owners, Admins, and Managers build and publish. Editors and Viewers fill Published forms; they do not get the builder.

1. Create a form

Open RequestsForms (campus workspace). Click New (empty state: Create).

The New form modal: Pick a starting point. Templates prefill starter fields on the same draft — you can change everything in the builder.

Starter templates

  • Campaign requestTitle, category, dates, copy, and images for a campaign. Use this when a ministry is asking you to promote something that should become a campaign (VBS, a sermon series, Christmas Eve). Title is already campaign-linked.
  • General requestSimple intake — title, details, deadline, files. Use this for a smaller promo that may not need a full campaign (a one-off announcement, a single Sunday mention).

Templates only seed fields. You can add, rename, or remove anything except the locked Title.

Multi-campus: Forms belong to one campus. There is no global form. Switch to the campus first. If you open a form from Global, you’ll see Forms are edited in a campus workspace and Back to forms list.

Forms are not tagged with a campaign category. Category is a question you can add (and Link to campaign Category) so it maps when you generate a campaign.

2. The builder

The first field is always Title—an Always on strip (lock icon + Submission title). It stays required and first. You can rename the label; the answer is still the submission title in Inbox. You cannot remove it or drag it off the top.

Field bank

Banks: Basics, Contact, Files, Layout. Tooltip: Click to add, or drag onto the form. Click a pill to append at the bottom, or drag onto an insert slot. Empty canvas: Click or drag fields here.

Dropdown and Checkboxes start with Option 1 / Option 2. Checkboxes can Allow Other. File upload presets: Images, PDFs, Documents (max 1–10 files). Page breaks show Back / Continue / Submit on the fill page.

Conditional logic

On a field, click Conditional logic (it stays collapsed until you add it, or opens if conditions already exist).

  • Show when — hide the field until a condition is true (answer fields and headings; not Title)
  • Require when — make it required when a condition is true (answer fields except Title; not layout)

Pickers: Choose a field, Condition. Operators: Equals, Does not equal, Includes, Is empty, Is not empty. Checkboxes use Checked / Unchecked. Each group is one condition. A hidden heading hides the following section until the next heading or page break.

Link to campaign

Not a separate bank. On a compatible field, Link (aria-label="Link to campaign") maps the answer into a campaign field when you later Generate campaign. Only one field per campaign key. Only the locked Title may map to campaign Title.

Contact fields, Single checkbox, Checkboxes, and layout fields are not linkable. Unlink with Unlink from {label}. Empty menu: No compatible targets left.

Notify and default assignee

  • Notify on submit: chips + Add person. Empty: No one selected. Clear all. These people get an email when someone submits—they are not the owner.
  • Default assignee: optional person who owns the request on submit.

Both lists are team seats only (Owner, Admin, Manager, Editor—not Viewers). Empty: No team members available to notify / assign.

Duplicate copies the form fields, not the notify list or default assignee.

3. Draft, Published, Archived

Builder: Draft shows Publish (after the form is saved and clean). Published shows Unpublish, Archive, and Preview. Archived shows Move to draft.

Archive confirm: New submissions will stop. Existing requests remain in Inbox.

4. Forms list

Search: Search forms. Managers see columns Title, Status, Submissions, Last submission, plus View and a kebab.

  • Row click opens the editor.
  • View (eye + label) on Published rows opens the live fill page. Draft and archived rows leave that slot empty so the kebab stays aligned.
  • Kebab: Duplicate; Draft Publish; Published Archive; Archived Move to draft.

Submissions is the open queue for that form (requests still Submitted or Needs info)—not lifetime total. Hover for total and unassigned. Click the number (when open count is greater than zero) to open Inbox filtered to that form.

Submitters and Editors see published forms they can fill—no Status / Submissions columns, no kebab. Click the row to fill.

5. After the first submission

The field bank and field details lock: you cannot add, reorder, remove, or change labels, options, mapping, or conditionals.

Bank: Fields locked after first submission. Canvas: Fields locked — title, description, status, and notify list still editable. Save toast: Saved (fields stay locked after submissions).

You can still change the form name, description, status, notify list, and default assignee.
